Should my student stay home from school?
If your student exhibits any of the following symptoms:
❏ Fever (100.0° Fahrenheit or higher), chills, or shaking chills
❏ Cough (not due to other known cause, such as chronic cough)
❏ Difficulty breathing or shortness of breath
❏ New loss of taste or smell
❏ Sore throat
❏ Headache when in combination with other symptoms
❏ Muscle aches or body aches
❏ Nausea, vomiting, or diarrhea
❏ Fatigue, when in combination with other symptoms
❏ Nasal congestion or runny nose (not due to other known causes, such as allergies) when in combination with other symptoms
Massachusetts Department of Public Health Guidelines Regarding COVID-19:
Important Reminder Regarding Medication At School: If your child is on prescribed medication during the school day all medications must be delivered to the school nurse in the original pharmacy or manufacturer-labeled container by you or a responsible adult whom you designate. A current doctor's order will be required to administer the medication to your child.
Reminder For Physical Examination and Immunizations: A physical examination of all students entering grade 7 or who are new to the school is required. Massachusetts law, M.G.L. c. 71, s. 57 and related amendments and regulations require a recent physical examination performed within the past thirteen months upon entry into school. Please forward a copy of your child’s most recent physical examination and immunization record to Silver Lake Regional Middle School. You can email it to, ndickinson@slrsd.org or juliehaley@slrsd.org or fax it to 781-582-3564. Your child’s healthcare provider’s physical examination form with immunizations is acceptable for our records.
Parents of incoming 7 students, please make sure you have submitted your child's most recent physical form with updated immunizations before the first day of school. This may also be faxed to 781-659-8822.
Grade 7 students will need:
- Hepatitis B - 3 doses
- Tdap Booster - Grade 7
- DTaP/DT/Td/Tdap - 4 doses DTaP/DTP or >3 doses Td; plus 1
dose Tdap - Polio - 4 doses (4th dose must be given on or after the 4th birthday and ≥6 months after the previous dose, or a 5th dose is required. 3 doses are acceptable if the 3rd dose is given on or after the 4th birthday and ≥6 months)
- Meningococcal - 1 dose, 7th graders
- MMR - 2 doses
Varicella - 2 dose or physician documented history of chickenpox disease
